By submitting the form you are agreeing to abide by all of the laws and rules of the Fraternal Order of Police and if your membership should be revoked or discontinued for any cause, you are agreeing to return to the lodge your membership card and any other material bearing the FOP insignia, such as auto emblem, lapel pin, etc.

Recurring Form: This should only be used in the month of September. This will automatically renew every year in September.

One-Time Form: This is a one-time dues payment in order to catch up using the recurring from in September.
